Requirements

1. Applicant must submit proof of insurance with application.

2. A county map with the route highlighted shall also be submitted.

3. We need an actual drawing of the layout of the weight distribution of the house that is being moved. This drawing is to include: dimension of trailer, tire size, tire air pressure, number of axles, dimension of axles, weight distribution on each tire, etc.

4. The actual dimensions and the weight of the home is also required.

5. Large loads must have escort vehicle and notify the Sheriff Department.

6. Each truck must be listed on permit. An additional sheet may be used.
